[1:06] proposed new building uh to the west side of the courthouse

[1:17] then around the corner and there when they round the corner there's going to

[1:21] be an x-ray machine and a metal detector and if somebody comes in the building

[1:24] and say they have a cell phone on them or a pocket

[1:26] knife they'll be asked to leave and that's that doorway to the straight

[1:30] up to the north end doorway b uh they'll be asked to leave exit out of

[1:36] that door there to go back around to go to their car or home or wherever

[1:40] to uh return that item that they're not

[1:42] allowed in with and they're gonna have to re-enter the building the way they

[1:45] came in to begin with um then once they proceed through the

[1:49] security area you kind of see they'll go through

[1:52] doorway d and then proceed east into the main building of the courthouse

[3:30] as well as the attachment of this building we're minimizing

[3:34] the historical attachment and disruption of the courthouse so we pretty much had

[3:42] a verbal green light to go ahead on with moodiness i was just going to ask

[3:47] what it's going to look like outside um is it going to be stone like the

[3:53] courthouse too that's what i thought it does have a stone veneer

[3:56] we are taking elements of the courthouse bringing it into this building so we

[4:01] have a large one a small one here a little bit

[4:49] it and with the landing the exterior landing

[4:53] and with that we needed to make sure that we had the height of this building

[4:56] up so you're not staring at the roof and we

[4:59] don't have people trying to jump up there

[5:01] as well so and then also we are taking care of that landing which is in dire

[5:06] need of repair so we don't have leaks coming

[5:10] in so we're looking at resurfacing

[5:14] preparing prepping and resurfacing the exterior as well as the interior

[5:19] so that's also on the agenda with these drones

[5:25] i think the main thing when you go back to the

[5:28] floor plan with the functionality of this building

[5:32] is the openness of it and the progression

[5:35] that they take to get into it and that also

[5:39] uh has to go with the exterior and that introduction to that little plaza out

[5:43] front which they're going to want to space out

[5:46] there let's create some space for them that uh minimize

[5:52] we're not talking putting terrazzo or anything like that

[5:58] concrete you know and by creating a nice plaza nice

[6:18] bit however uh we are proposing bollards in there

[6:22] removable bollards so that way vehicles won't come in we

[6:26] can stage them off um for security but we also have it for

[6:31] maintenance maintenance was a big thing so uh you can get it onto the

[6:36] lawn and mow and do some things as we're grading this

[6:39] out and for the proper slopes so get in and

[6:45] the other thing is secured all the way around this building is what we're

[6:48] looking at and that that comes into play with the landscaping as well

[6:52] getting rid of some of the shrubs low vegetation

[6:55] in there so people can't hike

[7:10] yes yeah because currently the only good mower on the south lawn is to put wooden

[7:14] planks on the steps and try to get the motors up there

[7:17] and even if we have to dig up a sewer line or a water line up front

[7:22] we can't get a piece of equipment up there so there's no way to access it but

[15:07] basically on page 33 shows the preferred signing for

[15:11] all the different zones one through five um like zone one is old highway 40.

[15:18] and that to me that was the most critical one out of all five of these

[15:22] was because it's it's unposted so it's 55 miles an hour with a

[15:26] curve sign for the west or the eastbound traffic coming into

[15:31] coming into town uh with a 25 mile an hour speed

[15:34] auxiliary plate underneath of it but um

[15:39] but uh they recommended the preferred improvements anyways

[15:43] basically to post that 40 miles an hour and after the existing chevrons upgrade

[15:48] turn warning signs and advisory speed limit plaques

[15:51] and remove the deer crossing signs so um zone two and if you guys have any

[15:57] questions as i'm going down through these

[15:59] please follow me zone two would be milford lake road near i-70

[16:06] currently posted it's not posted so it's 55 miles an hour for

[16:11] vehicles going over top of the overpass i-70 overpass and uh

[16:16] the recommended install install the posted speedup size 45 mile an hour for

[16:20] each approach

[16:24] zone number three would be milford lake road near liberty hall road

[16:29] their recommendation is to remove the posted 40 mile an hour speed limit signs

[16:33] and speed limit head signs and basically revert it back to a 55 mile an hour

[16:37] speed limit and zone four the old milford road

[16:47] that one there basically for a short stretch change the posted speed limit

[16:50] from 30 to 35 so increase at five miles an hour um

[16:55] and that's basically east of the first residence or

[16:59] over there by the golf course from there east would be increased up to 35 miles

[17:04] an hour from 30. then zone 5 would be hillside drive

[17:10] they recommended the install close to 35 miles an hour for

[17:14] southbound on northbound hillside drive traffic and install

[17:18] a winding road ahead sign the w15r

[17:24] what is it it's not it's it's not if anything is not posted it's

[17:30] 55 mile an hour but they still have to drive at a reasonable proven speed

[17:34] so we've had a resident after complaining about people speeding and

[17:38] he'd like it to 25 but i told him they recommended 35

[17:56] something like that but those were the recommendations for

[18:04] these five different zones that i submitted the application for

[18:08] and then back on that page

[18:26] page four do you guys have the basically spreadsheets in there

[18:31] showing how much the signs are anticipating being the cost of all

[18:35] the songs

[18:38] towards the back yeah the they've got it broke down for the cost of all these

[18:43] signs supposed to be installed and for the

[18:46] their estimated cost was 13 107 um and i when i seen that i'm like that

[18:52] is way high um so marshall went out and got

[18:57] three different quotes all the signs and which include the

[19:02] posts and newman came in at 3493.45

[19:12] and then national sign company came in at four thousand nine hundred and eighty

[19:15] dollars and twenty cents and then barco municipal products they

[19:20] came in at five thousand eighty two dollars and forty cents

[19:24] um but we have all of these signs in stock we'll

[19:30] probably just have to order a few posts so

[19:32] as far as out of money out of my budget to pay for all these signs we've already

[19:36] got in stock and marshall fields we've got

[19:38] an adequate supply so we don't have to reorder any at this

[19:42] time so i would recommend to the to the board to

[19:47] approve of this traffic engineering assistance

[19:52] speed limit studies for all five zones and for me to get the signs changed or

[19:56] installed

[20:58] an application so i did so and with the new construction at the

[21:02] high school there's some concerns out there on ritter road that

[21:05] you know the speed limit now is posted 45

[21:08] uh at least have an engineer look at it and get their assessment on what the

[21:11] speed limit should be if it needs to be changed lower

[21:18] and i did submit those applications to kdot on may 21st so

[21:22] waiting on them and i know it all depends on

[21:25] their funding uh for that program when it or if it gets selected so but

[21:31] i'll keep the board in the loop on my progress there

[21:41] next item be the cloud county community college parking lot island island sealed

[21:46] bids i again sent out bid packets to local

[21:51] contractors and the sealed bids we opened up here at

[21:55] my regular meeting on june 21st at 1 15.

[22:01] and so far it's not looking good it there's a there's a

[22:05] mandatory pre-bid meeting that they have to attend and i haven't got

[22:09] i haven't heard anybody that's reached out to me or dale

[22:12] uh to go out there and uh

[22:16] i've had one contractor call and they're just too busy so they're not gonna bid

[22:20] it so still and we've called them and asked

[22:23] them hey just to remind you this the deadline's coming up but i hope we

[22:27] get some competitive bids in for it's not looking

[22:44] discuss the uh

[22:47] lines creek east bridge replacement project got a couple change orders

[22:52] had some additional dirt work to do back uh behind the abutments uh because the

[22:58] original abutments you remember here my my last meeting said that they

[23:02] had the developments were a lot larger than what was anticipated

[23:06] well with that that those larger abutments there's a little void there so

[23:09] there's additional calculations and quantities of dirt

[23:12] filled dirt that wasn't calculated in for the contractor

[23:15] so they do these change orders so we made a deal with them to where we'll

[23:20] supply the dirt we'll haul you the dirt and that will help that drop that cost

[23:24] of that change over considerably but their first change order the cost

[23:28] for that filter was two thousand uh six hundred seventy seven dollars and

[23:32] fifty cents and if we didn't supply that dirt we

[23:36] were probably looking anywhere from eight to ten thousand dollars

[23:39] so uh we was able to to save save quite a bit of money there for getting them

[23:44] some some good field dirt that passed uh call

[23:47] valley's inspections on their testing so it tested very high uh in compaction so

[23:54] they were in which that has been done already

[23:58] and then the second change order would be for the piling

[24:03] um basically the short explanation is the steel pile must be driven into

[24:08] computed design bearing depths and the basically the pile had been

[24:15] driven down further in order to get into the good bedrock

[24:19] so um you know what they they they did the

[24:22] geotech investigation as far as core drilling to see what was down there

[24:26] um but upon findings they needed to add more piles so we we have another change

[24:31] order for the cost of additional piling and that

[24:35] cost was three thousand five hundred and four dollars

[24:43] i'd recommend to approve both change orders

[24:52] come over here we can change lawyer both

[25:12] he's literally looking for the numbers to receive you'll be proud of it

[25:17] yeah but they're the same yeah um and i guess another i guess

[25:23] in the future here coming up for too long i'm gonna have change order number

[25:26] three which is rip rap when again it's kind of like a domino

[25:31] effect the size of those abutments is going to affect this it's going to

[25:34] affect that so the riprap is going to be placed on

[25:38] in front of and down underneath the abutment and on the wings that's going

[25:41] to change a little bit so we're going to have

[25:42] additional rip wrap but we are supplying the rip rap

[25:47] from the rip rap that we're getting from fort riley we're going to be supplying

[25:50] some of that rip wrap for that so the cost of materials is not going to be

[25:54] there it's just the cost of the labor for the contractor to

[25:57] get everything put in uh and i'm anticipating it probably

[26:00] about the same cost as change order number one with the dirt

[26:03] so uh again with us supplying the materials it just drops that cost

[26:07] tremendously so and it's not just the materials but big

[26:11] shares the trucking and the hauling getting the materials on sites

[26:15] so just a heads up on change order number three

[26:54] 16 2020 gary county in turn had to close

[26:58] our facility to the public on march 16th um because we had nowhere to go with it

[27:04] on may 18th for riley opened their facility back up with restrictions that

[27:08] we must quarantine all of the recyclables for

[27:11] seven days and the only way we could feasibly do

[27:14] that is just have our operation open from monday through wednesday

[27:18] and be closed thursday friday and do not have a recycling trailer out

[27:22] at the transfer station um but uh last week i emailed uh herbal

[27:28] with fort riley and in talking to him uh to where all

[27:32] their employees from what he told me they've been vaccinated

[27:35] so he felt that we don't have to quarantine our

[27:39] recyclables anymore so that's why starting today we open back up to normal

[27:43] operational hours for recycling so it's monday through friday and then

[27:48] we'll also have the recycling trader out at

[27:50] the the transfer station like we did prior to cobid so

[44:18] wichita that we're able to get and then it's like okay we need to find a truck

[44:21] driver and somebody to wire it so we're looking

[44:24] at a day to two days for all that to happen well

[44:27] they made it happen overnight truck driver went picked up middle of night

[44:30] dropped it off the electricians came during the night and hooked it up for us

[44:35] so we got back to the business the next morning so we

[44:40] but we still have a generator issue so the problem with this when it's a

[44:44] mitsubishi it's made in japan and it was outdated so they don't make

[44:50] the parts anymore so they don't have any parts in japan they

[44:54] don't have any around here so we either have to rebuild this find

[44:58] someone that can build a starter for these or buy a new one

[45:03] jeff reached out to a couple places one place came back with a new generator

[45:07] basically like a caterpillar type diesel generator

[45:10] is like 500 000 one of our board members asked about

[45:15] what about natural gas well those are like four or five times

[45:19] that cost so i don't think that's an option but we

[45:23] still have the backup generator releasing for twenty thousand

[45:26] dollars a month so why did that go

[45:32] when they bought it oh i don't know it's before frank early 2000's

[45:36] yeah i call it bf before frank so i don't know what what you know

[45:41] sometimes you look at the cheapest thing you'll get or

[45:44] yeah yeah you do so um anyway so that's the generator issue

[45:50] so we're working through that the backups work and we tested it the

[45:54] fire marshal came someone complained that hey these guys

[45:58] had a generator you should go check it out so he came and

[46:01] he basically looked at everything and said you guys followed your procedure

[46:04] and you did the right thing complimented us for taking action for

[46:08] patient safety i'm glad it didn't take so long because

[46:13] we're thinking it's gonna be two days at the best at that point and

[46:18] end up being about 24 hours appreciate it yeah so and to the

[46:25] fire marshall bless this all we have some to send them back

[46:29] so that i think it's just an ongoing issue

[46:32] the other issue we're working through and wade mentioned this at the last

[46:36] board meeting but i don't think every anybody understood what

[46:40] the ramifications are we talked about pass-through payments

[46:44] uh we we basically are a demonstration project hospital

[46:51] for inpatient medicare and swing beds we get cost-based reimbursement so you

[46:57] guys heard that before we talked about that so the way they do

[47:01] that the operational piece of that is medicare pays us so much money

[47:06] uh a month like every two weeks we get a check for medicare and it's for this

[47:10] cost-based reimbursement at the end of the year they

[47:14] evaluate do an audit and say okay we didn't pay you enough we owe you

[47:18] or we paid you too much you need to pay us and there's a settle up or true up

[47:22] after the year and there's true i usually go drag on

[47:25] for a couple two more audits to be sure and then it

[47:28] finally finishes like we just finished 18 and 21.

[47:32] so it's usually about three years so what has happened is

[47:36] the demonstration project was renewed by congress and the president signed it

[47:41] well somehow our payments were turned off

[47:45] and it's about 200 000 a month 180 200 000 a month it's about two

[47:51] million dollars a year so we have reached out to the two

[47:55] seniors uh marshall and uh

[48:01] moran we've reached out to wps who's our mac administrator medicare

[48:06] administrator who gave us a contact with cms and cms

[48:11] has to turn it back on so we've alerted everyone that we need

[48:15] this term back on because it was approved to go another five years

[48:19] through 26. so we are waiting for that we even

[48:23] reached out to kha they have a person who's who's a

[48:27] liaison with cms we've reached out to everybody and said this is the situation

[48:32] so we've been working on that for the last week and a half two weeks of trying

[48:35] to get that resolved but you know if you

[48:39] take away two thousand dollars enough that's huge

[48:42] for us who would barely you know paycheck to paycheck company

[48:48] it just started on 26th yeah may 26th so it just happened

[48:54] so we had only one now we think there may be a glitch

[48:58] when it they renewed it and so where are we trying to figure out i reached out to

[49:02] other hospitals who were in the demonstration project which is about

[49:05] 30 in the country maybe a little less but the ones i've reached out to they

[49:10] didn't have any issues so we're trying to figure out why it

[49:14] stopped get it turned back on so if you know any of the senators

[49:19] representatives that are in congress congress has to reach out

[49:23] to cms so we've reached out to them and i haven't heard back today

[49:28] so that's what we've been working through and the holiday last week made

[49:32] it a short week for them uh they finally got to us like thursday

[49:37] friday what would you say um medicare services center for medicare

[49:43] services uh basically all the medicare is

[49:47] controlled by cms under dhhs department of health and human services

[49:52] at the you know the national

[50:00] so those are the the hot points we've been working on

[50:04] meanwhile we had a a year end so there was a lot of catch up and we don't go

[50:08] through the financials we had a lot of catch up to do and

[50:11] invoices need to be paid we ended up uh about a million dollars

[50:15] better this year than last year we generated more revenue than we did last

[50:18] year so in the time of covet and although we had

[50:22] decreases in clinic volume and er remember i share with you guys the

[50:26] volumes and how they were down um even with that

[50:30] we still made more money than we did last year so the house

[50:33] the way i look at it we were so bad and so lean

[50:36] the only place to go was up so we generated more revenue

[50:40] you know then we than the losses we had from that volume so i think that's a

[50:46] good thing part of that's the icu take care of the patients that payments

[50:50] a little higher so that's kind of the way

[50:56] next time clinic started that's generate revenue 340b

[51:00] uh but those things didn't get started until then really the one click may have

[51:05] had like a month the last quarter um i thought 340b had

[51:09] like the last quarter of some preliminary fun but those things are

[51:12] starting to look really good names any questions from me or any of that

[51:18] stuff

[51:30] yeah i did send the information about our equipment needs as well as physical

[51:37] needs for the plant that we've we've given to you a number

[51:40] of times but it's updated i sent it to was it deborah rebecca

[51:46] yeah you got that okay just make sure yeah

[51:50] and then um thank you yeah it's a way to go tv this is all

[51:57] this is all in the packet that he gave us but he made it easier for him

[52:04] so i've got the stable packet and the first page on that

[52:08] is the patient volumes and utilization statistics

[52:11] and since we are at a year-end i'm just really going to focus on the current

[52:15] versus the prior not really worry about the budget at this point because we're

[52:18] really looking at two different years the other part of

[52:21] this is not so much in the the volumes when we

[52:25] get to the financial information still preliminary because there's we

[52:28] have our audit coming up at the end of the month and there will

[52:32] be some adjustments that will be presented with the uh that

[52:36] will and will most likely improve it so

[52:41] uh the highlighted areas you can see the acute admissions are

[52:45] on the top line there and if you compare that to prior years up a little bit

[52:48] um so we did acute admissions are going to be those in

[52:52] the medical surgery as well our medical surgical

[52:56] area as well as icu so those areas were up a little bit

[53:00] uh surgeries you can see were down that was typical for a coveted year

[53:05] particularly during the height of it probably last

[53:08] summer would have been the bigger areas when those surgeries would be down

[53:13] births uh we did um experience more births this year

[53:17] again covered here and that's been kind of a nationwide trend

[53:21] uh the lockdowns and such emergency visits

[53:24] uh emergency visits were down from last year about 8.2

[53:28] percent uh to give you a little bit of insight on emergency visits we do have

[53:34] our physicians are contracted and in that contract

[53:40] the higher the volume the cheaper our contract

[53:44] is feels a little bit weird but because they're able to build for the

[53:49] uh their component so it reduces the amount so um

[53:52] the difference here which was about a thousand er visits

[53:56] cost us several hundred thousand more dollars in our contract

[54:00] so uh just as a side note there uh outpatient diagnostic diagnostic

[54:05] physics those were up you would think about

[54:08] the laboratory and imaging in those sort of areas

[54:11] uh lab clearly was going to be up clinic visits those were down 18.5 this is

[54:17] different from the rural health clinic visits

[54:20] so the clinic visits would be the orthopedics in the surgery

[54:25] uh which is structured differently for um

[54:29] structured differently in the build a little differently so they were down but

[54:32] the rural health clinics were also down by about two percent uh we did add dr

[54:37] corey's practice in the month of april which did

[54:41] improve it a little bit but um

[54:46] so there's only one month in there next page wound care procedures didn't

[54:52] want to point that out you can see we've had 180 for the last

[54:57] two months so

[55:02] i'm going to push back to the page four it's going to say operating performance

[55:07] summary

[55:10] and um again focusing on the fiscal year to date you can see our gross patient

[55:15] revenues were 110 uh 110 million versus 96 of last year

[55:20] but if you look into the details on that a little bit

[55:24] you can see the impact for our inpatient revenue uh it's up 30 almost

[55:29] 31 percent now we did look at the admissions

[55:33] that were up just slightly however we the patients that we had this year were

[55:38] much more severe as you would expect with code so there would be sicker

[55:42] individuals and it would be longer stays more charges associated with that so

[55:46] that drives the revenue up we did have more outpatient revenue

[55:50] as well again you have lab that was up in diagnostic

[55:54] areas uh total deductions we were up about

[55:57] 16.8 percent that would be expected if your revenue was up uh tax

[56:02] subsidies in there our other revenue this is the area that will

[56:07] be scrutinized in our audit and everyone's would be

[56:11] uh it doesn't has our covenant-related dollars in it so you can see the 6.9

[56:16] versus 2.3 now that's not everything we received

[56:20] we do track items that we potentially could pay back on our balance sheet so

[56:25] these are the items that we uh have utilized and

[56:30] put into the uh the income side of things

[56:33] so it is up a little bit on this side a net operating revenue of 40

[56:37] just 40 almost 40.5 million versus 33 up 22 but then again our expenses would

[56:44] also be up and you could see 43 million uh for the year compared to

[56:49] 37 last year the bottom line on that one the 4.3

[56:53] million is going to have a lot of uh coveted related uh dollars associated

[56:58] with it so salaries and wages were up as well as our professional fees and

[57:04] some areas there that were up as well the operating

[57:08] margin for the year on 3.1 versus just almost

[57:13] 4.1 last year so about a million as franklin

[57:15] mentioned earlier um total net margin you can see that there and the ebitda or

[57:20] the earnings before interest and depreciation amortization

[57:24] was a negative 691 compared to 13 and 1 million

[57:28] 394 last year so some improvements there

[57:34] uh i'm going to dig back further in the report now it's going to be page

[57:39] 12. so much of this is statistics and or trends and things but we're going to go

[57:45] all the way back to the balance sheet so that you can see what we have here

[57:52] in the highlighted area here is days cash on hand

[57:55] it's at 42 you can see that last year and we were at 70 minute

[58:00] last year and we were just getting those funds

[58:03] we were a little bit higher last month as well so

[58:07] there are some things that are impacting our cash flow one is the

[58:12] uh going forward is the password payments the other one is

[58:16] uh and actually let's see now yeah let's go ahead and cover that

[58:21] now this other sheet right here that i handed out as well um so you can see on

[58:28] this and it starts with that medicare accelerated payments

[58:31] the intent that program was just to basically get cash into the

[58:35] health care facilities and though they're advanced paying for services

[58:39] that would would were to come and then

[58:43] at some point it was that we would have to pay those dollars back and you can

[58:47] see that we've paid we're at the very early stages it

[58:50] started in april uh 200 and almost 215 000

[58:55] so last month alone we had 140 000 that would be may so when you

[59:01] start thinking about capacitor payments that we're not getting which is about

[59:04] 200 and then you have an additional 140 going and you can see

[59:08] what that does to our cash flow so that's a little bit of a stress right

[59:13] now we do also have the sbea small business association for

[59:18] givable loans also known as the payment protection program we did

[59:22] receive multiple funds in to the total of 6

[59:26] million we've applied for forgiveness for

[59:31] all but two million is two million we are not ready to apply for that one yet

[59:35] and we just haven't received the notification that that's been forgiven

[59:38] yet i know that smaller organizations churches and smaller businesses have

[59:42] gotten those letters um so we just haven't therefore it's not

[59:46] in the revenue because we had don't have that letter uh

[59:50] the cares act the provider leave funds we've received a significant amount here

[59:54] now uh any excess in this is subject to

[59:57] payback so this is the area that we talk about

[1:00:00] lost revenues and you can see that we have better revenues so that that

[1:00:04] doesn't really apply to us in too much

[1:00:08] it does to some degree because there's other methods in what you can calculate

[1:00:12] that as well as expenses so the the

[1:00:16] supply expenses or the time um included in this we did include the roof

[1:00:21] but that's in a really gray area we may get denied on that one

[1:00:26] so at this point we've recognized a number of dollars

[1:00:30] but uh there's still more work to be done on this we're

[1:00:35] we're going through additional areas and items that we've already expended to see

[1:00:40] okay can we justify or find an argument to where we can

[1:00:45] uh keep some of those dollars so that's one area we do have some other

[1:00:51] grants in here as well that we were able to utilize

[1:00:55] the payroll tax deferral that's another program that basically

[1:00:59] we didn't have to pay our we calculated but we didn't have to pay our payroll

[1:01:04] taxes for our time uh we just got notification i think in

[1:01:07] the last two weeks that we will end up paying all the employees

[1:01:12] portion as well as half of the employers portion by the end of this year

[1:01:16] and then the remaining employer portion by the end of next year

[1:01:20] sparks grants that went to operations and then we have a couple of other

[1:01:24] smaller items here that are also subject to uh payback but where

[1:01:28] this is kind of a work in progress i know with the cares act um there is a

[1:01:34] timing issue on that and uh

[1:01:39] we're hoping for that to get pushed back but the cares act funds

[1:01:43] are supposed to be expended by the end of this by the end of june

[1:01:47] so they would have to be fully expended uh at that point

[1:01:51] there is some discussion whether or not that's going to get moved back to the

[1:01:54] end of the year we don't know so we're just preparing for

[1:01:57] that coming up as well so when we were looking at the

[1:02:01] um balance sheet we had about six million dollars in cash so

[1:02:06] it's really to our benefit if we can protect as much of that going back

[1:02:12] but that's still subject to how cms or medicare

[1:02:16] views our justification and with the cares that sponge you guys

[1:02:23] can't use that money towards uh the generator

[1:02:29] well you can always apply for it but it's gonna

[1:02:32] it would be a really great area because the intent of that was for

[1:02:37] items that are directly covered related so to speak

[1:02:40] so the fact that the generators started and

[1:02:44] that wasn't because of code that was a routine operational thing that we've

[1:02:48] been doing or we should be doing every week for

[1:02:51] years that's something we test those generators that's not i mean

[1:02:57] if we don't have it we can't treat a patient i understand that but it's not

[1:03:01] really my guess is cms would deny it but

[1:03:07] who knows we can always try have you looked at the guidelines for

[1:03:13] the american rescue monies the way i look at

[1:03:17] it i think that we can use that money towards maintenance of the hospital

[1:03:20] yeah it appears so yeah i've looked at it a few times and it's

[1:03:26] there's a lot more gray than the previous yes so

[1:03:42] i feel like you got you overhead you guys never put your

[1:03:48] lights off never like you never stop operating

[1:03:53] and so i just you see that specifically it's weird

[1:04:00] sorry i know i feel the same way it's a head stretcher yeah yeah other

[1:04:06] things we bought ventilators and oxygen stuff that's all

[1:04:10] direct and we can apply that loss of any revenues time

[1:04:15] put in place to get ready prepare and test and lab and all that stuff

[1:04:21] applies and yeah we could spend it and pay for the roof and pay for a

[1:04:27] generator and then apply it and it may or may not then you may have to

[1:04:31] pay it back in at the end so was it judicial to do that

[1:04:35] or not that's how long does that take time you could not

[1:04:55] so the portal's not even open yet so they haven't

[1:04:59] built it so that's part of why we're hoping that the

[1:05:03] the expense of the funds can be pushed back as well so we don't even know what

[1:05:06] the questions are going to be at this point

[1:05:09] so but that's kind of routine with

[1:05:14] um cms to some degree oftentimes they'll set deadlines and they'll get moved back

[1:05:18] until they finally get it where it's to put out what we

[1:05:22] call a final rule and then when that comes out then that's

[1:05:25] the the bottom line

[1:05:31] don't ask me that question no it's okay yeah and wait can you

[1:05:38] because i knew i couldn't explain it but i i did

[1:05:41] talk to them about um we had mentioned at the meeting about

[1:05:46] refinancing the bond and right so the the there were some issues

[1:05:54] actually i think that's something that's with me okay i thought you said that

[1:05:57] little notebook so we um we uh did have another

[1:06:03] conversation let me just find this real quick

[1:06:15] there we go with uh kevin uh so there there was some some issues or

[1:06:22] some thought processes with um that at least that's what we were

[1:06:28] looking for was what options were there um i know what

[1:06:32] the larger county option there's a timing as well

[1:06:41] there's a call ability and i don't think they're in the uh

[1:06:46] they're in the window yet they're not callable yet

[1:06:49] uh and so if we were to if something was to be done now then it would be

[1:06:53] it would change the taxability for it so uh there may be if there's a tax law

[1:06:57] change or something like that that could make it more viable uh but

[1:07:02] these it's kind of a back burner item um some refining so there's some things

[1:07:08] that we could do potentially for the facility expenses you can kind

[1:07:14] of say okay well how much do we spend and

[1:07:16] re and issue a new one um the other parts with those public funds

[1:07:22] is before capital improvements that's kind of what that was

[1:07:25] uh there would be an amendment for the supplemental lease

[1:07:30] there's a bond issue and for there's also a number of

[1:07:34] registered voters that have to be part of a vote to

[1:07:38] um once it's been in the intent has been published and there's a 30-day protest

[1:07:43] period and then you need five percent registered voters

[1:07:46] um you could put together a new resolution

[1:07:50] that's a 60-day protest and then the lesser of two percent of voters are 100

[1:07:54] electors so there was he was really looking not so much at the

[1:08:01] bigger bond in those on that front so

[1:08:07] the bottom line i had he said it really would be ready to even look at that

[1:08:10] like 20 27 okay from the timing standpoint

[1:08:15] and then he did look at the smaller bonds that we have he said

[1:08:19] not refinance but refinancing a payment or moving one payment like the next

[1:08:23] payment to the end of the cycle there's process to do that

[1:08:26] if that's what you choose to do are you good it's like about two hundred

[1:08:31] and fifty thousand dollars that's it yeah

[1:08:37] it moves the principal payment yeah yeah just keeps and then

[1:08:40] and there's some tax pieces to that so then you'd be taxable

[1:08:47] it didn't seem it didn't feel like it was worth it to do that

[1:08:51] piece of it unless we got to where there was no other options

[1:09:32] been a little bit busier so that feels like kind of back to normal

[1:09:36] last month and end of may we had around a little bit over a thousand page visits

[1:09:41] which is the first time we broke a thousand like

[1:09:43] seven or eight months so that's coming back

[1:09:47] alpha care we're extending hours that'll be coming up

[1:09:51] shortly to coincide with the urgent care that's coming so we'll have

[1:09:57] those extended hours

[1:10:00] we did um also challenge that electric bill i think i

[1:10:04] told you guys it was like uh 30. again excuse me guess bill

[1:10:09] 130 000. so um we did challenge that heard of a payment

[1:10:15] to satisfy and we'll we're still working

[1:10:17] through that to make sure that that's going to work so hopefully that'll be a

[1:10:22] positive you don't have to spend as much yeah i think it's a bunch of individuals

[1:10:27] and other entities going challenging their their

[1:10:32] bills so yeah the biggest thing i could say

[1:10:35] about if you look at last year we had about the same amount of

[1:10:39] inpatient visits in patient days but we generated a lot more revenue

[1:10:44] because they were sicker i feel like i think we call the case mix index and

[1:10:47] that was all the cope with patients that we had we're taking care of

[1:10:51] i think we had about 80 of them maybe a little bit more now

[1:10:54] uh 90 patients that we took care of so that that helped generate the revenue

[1:11:00] and that was the icu and having all those services available

[1:11:07] did you bring your um list of accomplishments oh

[1:11:10] gosh no i didn't know well i didn't bring it to them

[1:11:14] you know because you want me to tell you we're gonna do it so i'll bring it to my

[1:11:18] monday then okay all right let's see what i think

[1:11:20] about well wound care yeah that's 180 visits 340b generating

[1:11:25] revenue we were a leap frog safety

[1:11:29] grade a only one in the region then you got to go to kansas city to get another

[1:11:33] grade a the second year in a row that's hard

[1:11:37] yeah that's awesome quality scores improve our patient satisfaction went

[1:11:41] from a three star to a four star so patients are satisfied at the

[1:11:45] hospital um so those are just some of the highlights

[1:11:49] the revenue i think you know just the morale seems a

[1:11:53] lot better what i hear because i only know some when i was here yeah

[1:11:56] prior years it seems like it's better yeah you

[1:12:00] have walked the halls so it's way better that seems better

[1:12:04] we added an intensivist that really made and we had a tele neurology and psych

[1:12:09] which is starting to really take off now nice so those things are are moving

[1:12:14] along we're added several contracts to trying

[1:12:17] to make it more like a regular hospital this hospital is like

[1:12:20] stuck about 20 years ago that's the way it was operating and

[1:12:25] we're trying to get it up to speed uh we still have a paper chart in the

[1:12:28] clinics we need to get rid of to get to an electronic system

[1:12:32] so we're working through those issues we've got a lot going on and

[1:12:36] our business plan i can present maybe next time the small

[1:12:40] highlighted version just so you can see it's basically about driving and

[1:12:43] building more revenue because that's what you need

[1:12:45] yeah i know you and i talked about doing some town halls

[1:12:49] probably wait till yeah let me present that to you and see what you think if we

[1:12:53] need to tweak and add and um that's the strategy is to grow

[1:12:58] revenue yeah i can tell you frank from the

[1:13:01] outside looking in if i didn't know what was happening
